On a regrind, they reduce the base circle (point of 0 lift) of the lobe to get more lift on the nose. Basically, they're just altering the position of the lobe in relation to the centreline of the cam. Be careful though, in most cases you need longer pushrods and/or adjustable rockers to make up for the loss of metal on the base circle of the lobe.
IMO, yes. I wouldn't use a regrind cam, unless it was a roller cam as they're a steel billet, even then i'd probably just buy a new cam.
