    What car?

You will need wiring harness, computer and pretty much the whole engine. I've heard of a few people doing this to their old X series Falcons, but they usually take the insides out of a donor and do a transplant.

The cost will depend on the vehicle. But find out the price to buy a wreck or a complete motor (with EVERYTHING) and if it all bolts up, you're good to go.

It shouldn't cost more than $1500 for most cars.

Yep depends on your car and motor, its also possible to just use the top end of the EFI, on to the old block, saves messing around with the new polution requirements. Thats for 308 only though

as said , depends on car/engine.

for the 308, you need heads, inlet manifold, exhaust manifolds, computer, wiring, fuel lines to/from tank and maybe different alternator/power steering barckets ( usually mounted on the opposite side to carbied blocks/heads ).

not sure what fords need apart from computer, wiring and fuel lines to from tank.

You can also do that with a 202 can't you? I saw a few years ago a guy who converted a blue 202 to EFI by using the EFI gear from a black 202 on his original blue block.

Sure can, im not 100% syre but i think you only need to fit the EFI manifold, and wiring etc, no need to swap between between blue and black heads though their is an advantage in using the black head. I may be wrong though you may have to use the black head, DANNY8 will know.
You can also fit the Blue/Black head on a red 202 you just need to drill whole for the Blue/Black water galleries using the B/B head gasket as a template. Fitting a B/B head over the read is advantage due to upgrading to 12 port instead on 9 port, also gives you more options to play with.
