Bad clunck and rattle when starting V6

Brother just showed me how when he turns his VN V6 auto on it rattles and clangs real loud then settles into a steady clunk when idling. I assume it has to do with the starter and flywheel like the starter cog is not disengaging. Like i said its real loud on start up like dragging an anchor behind your car... Yeah anyone had a similar prob?

I have heard a main bearing in a couple of motors that were shot and this didn't sound like that at all. Sounded outside of motor, too loud to be inside.

Turned out to be starter not engaging/disengaging properly, had a spare and it sorted it out for now...
