Fuel issue

Hey guys, i have done a fault check on my VP V6 commodore and got code 12 so everything is all good.

Now i have replace fuel filter, spark plugs, leads, air filter, have a free flowing exhaust and extractors, cat isn't collapsed and im still not getting anymore than 300 km's out of a full tank of fuel.

I need some ideas what it could be?

Could it still be the O2 sensor even though i didnt get an O2 sensor error code???

Hows your gearbox? I tried everything with my vr wagon 3.8l but still never got anything over 300kms out of a tank, I ended up selling it as I think it was a gearbox problem, the box was slipping and holding gears for too long, reving too high that could cause heavy fuel consumption.
