Car Dies at high revs coughs bad fuel?

So i was driving last night and my car started to cough and die high in revs underload, low revs you cant hear it but lacks power still not as bad as high revs and its been getting hot, checked water levels and only down abit so i topped it all up but i dont know whats up with it, i pulled spark plugs out and replaced them with others. at idle sounds like it is missing abit, but tis always had abit of a miss, i checked all vaccum and its fine.
I put fuel in that night and i thought it could be a bad batch of fuel, because it sounds like the car use to when the fuel reg oring went and would only do it at high revs.

I checked error codes and tis just coming up as 12... what could it be CAS is new,exhuast sensor new. Coil pack? Leads?

BTW i put in premium. 98

anybody got any info or has had it happened to them at some stage, its a VN V6 N/a.

Are the injectors clean? Clean fuel filter? Sounds like there "could" be a blockage/s somewhere along the line.

found out the problem..... stupid lil thing, when i replaced the engine mounts the neg lead from the battery to the block the bolt was loose and must have got looser, so at high revs it will wobble and rattle around and not give a good earth so my starting problems have stopped and so has the cutting out problems,
