Rear wheel hub replacment, or bearings, loud hum noise when turning at 50+ km/ph

Okay so check this out.
I got a few lil problems with my car at the moment.
1996, VS, Executive.
Lets say im going 40k's an hour.. well when i brake and slow down. Just before i get to a stop it makes a crunchy sound. I feel as if the wheel bearning in my back left wheel has gone bust. If im in traffic and i idle forward and brake, it will make this sound. It sounds as if there were rocks in my bearings instead of ball bearings. So its like a rough scratchy sound.
If someone knew what it was, i'd appreciate it if you could tell me, and then tell me how to fix it? Thanks.

Also, when i go at about 50+ k's an hour.. if i turn left on like a 10 or 20 degree angle. it makes this loud humming sound. almost like something is rubbing really fast against something else. its a kind of high pitched humm noise.
Any help would be muchly appreciated!
