I assume you have done the basics such as clean the IAC, check codes etc.

I would look at cleaning under the DFI and the bracket to the cylinder head to ensure a good earth. Make sure there is enough zinc oxide grease under the DFI so the heat sink is effective.

Maybe a earth wire was damaged when the exhaust was fitted?? Look for this also.

Have you checked the O2 sensors? After fitting extractors the coating on the inside of them contaminates the sensors and can cause a bad idle.

The pacemakers also fit the o2 sensors slightly further away from the engine than then stock headers, so together with your better flow from the exhaust and further distance your o2's will be reading differently than when they were in the stock headers, so your idle may become slightly ruff as the MAF calibration will be out by 7% or so, your ecu through blms with your o2 sensors will correct for this over time but it still wont be perfect, a tune and MAF recalibration can smooth out the idle and power curves again, i have done this on a 2 vs's now and the idle has been much better, im not an experienced tuner though like others here.

As above first things to check would be o2's which can also die after installing extractors as the internal coating wears off, IAC may also need cleaning and resetting.
