I'd suggest doing an error code check to see whats going wrong. You shouldn't be touching the accelerator at all when starting though, no need to do that on EFI cars as it can actually make the situation worse. If you are getting a puff of black smoke, the car is running too rich, so that could involve oxygen sensors, but they aren't in use when you first start the car.

I would be looking at the coolant temperature sensor (the one that sends info to the ECU, not the gauge). This may be telling the ECU that the coolant is colder than it really is, and the ECU then compensates by richening up the fuel mix, and that will give you your hard starting because the engine is basically flooding itself when you are trying to start.

Couple of other things to look at could be faulty or blocked injectors, buggered fuel pump and or fuel pressure regulator. Also maybe replace your thermostat, it could be stuck open, which will also cause the engine to run too cold when you are normally at operating temperature.

check the throttle body and IAC motor are clean. you can remove the IAC by undoing the 2 screws and remove it. spray it with throttle body / carby cleaner , also spray it thought the throttle body itself to clean the butterfly. Don't adjust the IAC while it is out, just clean it and put back in. And DONT TOUCH THE TPS. The IAC valve meters air to the engine at startup and idle, so i reckon its blocked, which is why you nedd to blip the throttle.
