Default vt v6 hesitation when accelorating

hi guys, i hope some one can help me. situation is i have a vt v6 calais which has developed a bad hesitation(like it's runnig out of fuel) when accelorating. it started out as a flat spot at full throttle at about 4000rpm but has gradualy got worse. now it's all through the rev range apart from light throttle. also if it sits there idling the revs flare to about 1400rpm and the drops back to 1000rpm(too high). when it does this it sets error code 36, which indicates a vacuum leak( which i cannot find by the way). this is what i have done so far - replaced fuel filter and cleaned tank, fuel pump pressure and delivery seems to be o.k.replaced i.a.c valve and cleaned throttle body, checked vacuum hoses, replaced tps sensor, replaced inlet plenumn gaskets( both bottom cover and throttle body). if i diconnect the battery to reset computer it runs o.k. for about 30-40kms and then the revs flare up again and it set code 36 again. this is the only code it sets by the way. please help if you can

I was thinking the same thing. Sounds like they might be clogged!
