vu power steering line burst twice

Hey guys.
I mate of mines power steering hose just blew off the resovour. It's has snapped the house clean off. What could this be? We change the hose and resovour and bled it and it happened again???

Which hose blew off the reservoir? The one going to the pump (lower one) or the return line from the rack (higher one)? Sounds like you may have a blockage somewhere.
