What does this mean-
code 57 is injector power supply monitor, voltage variation at ECU terminal A4.

My manual only shows DTC57 applying to Holden V8 engine with auto trans - is that correct for your car? Not sure why this is so I would have thought it applied to manuals as well. The description is:
The injector voltage monitor line is used so that the PCM will know the exact voltage the fuel injectors are operating at. This voltage is used to control the pulse width modulation of the fuel injectors. If the injector voltage monitor line drops more than 2.5 volts for more than 5 seconds, Diagnostic Trouble Code (DTC) 57 will set. This DTC will turn ON the Check Powertrain lamp, and will have a DTC set in the PCM......
You need to check the PCM connections for proper mating - C15 and C16 are for injector control and D7 is for the injector voltage monitor line. Don't know where you got terminal A4 from.
