Default Alternator Troubles?

Just tried to start my VY V6 today and it wouldn't go. Voltage read 11.9V. Got a jumper and started ok. Now when I check voltage with load (High Beams, etc.) I'm getting 14.25V @2500RPM. The workshop manual states 14.35 - 14.65V from the alternator. Anyone got any idea what this could be? Worn alternator brushes maybe?
The car has 155K on it.
Also, battery is about three weeks old so it shouldn't be that.
Cheers

All connections are good. I just don't know whats causing the battery to go flat. There's no load after the car's turned off and the alternator's only undercharging very slightly.
