Check your oil, when was the last time it was changed. Also check the diff mount bush - cheap replacement, they are usually the culprit to diff clunks.

My diff guy says most VZ's and some VY's have diff clunk and usually comes from excessive backlash throughout the whole diff drive drive train, some people say its the gear set while others say its the axles but supposedly it won't hurt anything.
My like yours clunks at slow speeds but under medium to hard acceleration it changes fine and smooth.
The one thing I can tell you for sure is if you replace the rear outer cradle bushes with urethane the clunk will get louder not quieter.
I used urethane when one of my bushes had worn out but ended up going back to rubber as the louder and sharper diff clunk was driving me crazy.

Well thats funny you say that because since I went back to rubber outer cradle bushes I've basically forgotten about it.
Still notice it at low speeds when don't have stereo on but otherwise I just live with it.
You could spend a lot of money chasing the causes as I outlined above and still have the clunk.
Personally I will wait until something breaks then spend the money.
I've had my car 4 years now and the clunk has been there all that time.
