normally its the hinge pin that has worn out/bent...they can be a pig to get out for replacement even with the correct tool....at holden they adjusted the door by lifting the car with the door full open in bad cases they used a jack with a wooden block
no idea re the crack maybe post a thread with a pic...any good panel beater should be able to fix it.....
