Unfortunately now that the photos are gone above, I went searching for a video on how to replace and rekey a Commodore ignition lock. The video says for VZ Commodore, however the instructions worked the same for my VS. Hope this helps others!

Thanks all, I found the issue - a weak spark on number 4 (ie lead wasn't on the spark plug far enough). I'd say that I never noticed it before the exhaust install, as I never usually drive the car that hard.
