Car struggles past 4000rpm and lacks power! Done everything besides the Dfi module and cat, cars got 280000km and it's manual ,how long do cats last?

Double check that you dont have any exhaust leaks, Iv heard that the gaskets need to be replaced otherwise ull have Somthing like a flat spot. Just a suggestion.

Does it do it at specific speeds?

Like most cats with 280,000k's on them, they will be blocked to a certain degree.

I'd check the fuel filter and fuel pump. A partially blocked cat will still rev, just won't be as quick to get there.

Sounds like a fuel issue, have you cleaned the injectors?
