Door Central Locking Issue

I previously had an issue with my Right Hand Rear Door Not Opening and needing the leaver to be manually lifted but after a few days it started working again. It has now been many months but now the opposite door has now got the same problem. However today I went to open it manually and it and the leaver came out with a metal thing on the end of it. I am hoping that I will be able to just put it back in but does anyone know how to fix this? and can I do so without removing the Door trim?
