First click user control panel (User CP), then profile options and you can add custom title's from there.

Yea it would seem moderators have the ability to at least do their own ones, hence Gobbles being 'The Pissed Moderator'

Nah I changed it to that as Darren has given us greater power (can do almost anything bar edit forum code) and vBulletin refers to this as a Super Moderator.
