Very brief Hesitation when accelerating over 4000rpm

I have had a problem with my car briefly hesitating in acceleration above 4000rpm, I doesn’t happen all the time. It happens when I flat foot it and when I very slowly accelerate to 5500rpm.
There is a bang and the engine gives off for a split second and catches back on.
My mechanic said it was my diff, so I got a 2nd hand one put in, so far the bang has gone but the hesitation stays.
It has been 5 months.
Car has 165k with OTR and Dino tune, bigger extractors and full exhaust, pushing 297KW.

My guess is something relatively simple, like a spark plug, lead or a semi block injector
