Yep, I just changed the pulley, and it is still squealing. Replaced the serp belt while is was in there thinking that might help, changed the idler pulleys as well. I guess I'll have a crack at the tensioner itself when I can source one.

I bet on the starter motor, probably has worn out, and different temperature makes certain metals sizes increase/decrease, and that could be the reason it only happens sometimes.
