Found out a bit more. When cranking, all the injectors are firing, once the engine starts and the PCM gets a good signal from the cam sensor then each injector is fired in the correct order. So it comes down to either a dodgy cam sensor or even the PCM. But if you are not getting a decent pulse even when cranking then it may well be the DFI.
