vicarious that is pretty much it, mine includes the last part for the metal clip though.

These pics are based on a lever I've already removed and from memory.
Step 1:
Leverage button from the top with a small flat head screwdriver
Step 2:
Remove Button
Step 3:
Remove screw:
Step 4:
Remove backing of gear shifter
Step 5:
Carefully remove centre part including a horseshoe shape piece of metal at the bottom
This is the lever in pieces:
Step 6:
Once the horseshoe piece of metal is out you should now be able to pull it up and remove it from the car.
Step 7:
Complete steps in reverse to put it back on and make sure the spring between the button and the centre matches.
